Electrothermal Therapy or Intradiscal Electrothermal Therapy or Intradiscal electrothermal therapy (IDET) is a non-surgical approach for patients with back pain caused exclusively by tissues of the disc. With age or injury, cracks or fissures may develop in the wall of the disc, causing the disc to bulge. Filled with small compressing nerve tissue and blood vessels, these fissures become a chronic source of pain. After examination of your MRI , diagnostic ESI"s, and a discogram (an injections of dye into the disc for diagnostic purpose) Dr. Ribnik will decide if your pain can be effectively treated by IDET. Patients are carefully selected for this procedure because pain from spinal joints will probably not be relieved. The IDET procedure uses an electrothermal catheter inserted through a needle which has been placed into your painful disc. A heating element within the catheter is then activated. The treatment lasts approximately fifteen minutes and the catheter is the withdrawn. The goal of the IDET procedure is the contraction or closure of the disc wall fissure, or a reduction of the bulge of the inner disc material. You will receive IV sedation and pain control throughout the procedure. Healing time for an IDET is 12-16 weeks. During this time you will need to wear a tortoise shell back brace for support. This brace will be custom fit prior to your procedure. Anticipated risks include infection, hemorrhage, allergic reaction to medication, and possible nerve damage. |
